RESUMO

Digital transformation has been making its mark on organisations in healthcare and beyond over the last few years, with no signs of stopping. In the NHS, much of the focus has been on acute services until recently, but this focus is now shifting towards community services, with Sussex Community Foundation Trust (SCFT) being named as the first community Digital Aspirant Trust by NHSx. This article explores what digital transformation is and uses SCFT's experiences to illustrate how this can provide benefits for community nurses. It considers what the future of digital transformation might look like and how clinicians can help to ensure patients remain central to any change.

Aim: To analyze the interventions of community and public health nurse in the context of health research and innovation. Method: A Systematic Review of Literature was conducted according to the Equator Network protocol, Systematic Reviews, CRD's guidance for undertaking reviews in health care, University of York. Results: A total of 23 articles were obtained, according to the inclusion criteria established. The qualitative analysis showed that references to interventions were reported together with others, although some articles focused only on interventions. We emphasize the qualification of the "leading nurse" in the approach to community health intervention, which is the driving force and responsible for taking initiative in future policies. A training program in public health nursing, prepared to be available on the internet that started in 2018, to establish new nurses in the area. The methodology of Item Response Theory in public health research to evaluate, validate and develop instruments, such as the Japanese version of a drowsiness scale. Conclusions: In terms of innovation / creativity, 2 articles specifically use the term. However, others, not mentioning any innovation, do in fact, as it is the case of the development of a new public health training program. As public health reform is taking place in Portugal, we believe that it is an opportunity for nursing to investigate, innovate and recreate, as the European Union recognizes that investing in research and innovation is investing in its own future

RESUMO

OBJECTIVE: present and discuss the potential use of the concept of vulnerability for the development of practices to address violence against children. METHOD: this is a theoretical study that presents the characteristics of the phenomenon of violence against children, to which the concept of vulnerability is applied. RESULTS: proposal of a framework for the analysis of violence against children in the perspective of vulnerability, creating individual and collective dimensions. Violence against children, when analyzed in the perspective of this concept, broadens the understanding of this phenomenon, as it exposes aspects and conditions from outside the family that are co-responsible for the occurrence of this phenomenon. CONCLUSION: developing studies to identify the vulnerability of children to violence is essential for the development of practices to address this issue.

El/la enfermero/a de familia y comunitaria es el profesional que a través de una mirada integral y holística asume la misión de acompañar a las personas desde su nacimiento hasta la muerte para desarrollar su potencial de salud, promocionando los diferentes entornos familiares, laborales y sociales para facilitar dicho desarrollo. A lo largo de la historia, diversos organismos de ámbito internacional, europeo y nacional, han ido regulando la figura de estos profesionales de enfermería de familia y comunitaria, hasta la fecha actual, donde existe normativa que regula de pleno derecho sus funciones, atribuciones y desempeño profesional. El personal de enfermería de familia y comunitaria puede dar respuestas a las necesidades de una población cambiante, y que asume nuevas responsabilidades en la gestión y la investigación. Sus amplias competencias básicas y avanzadas recogidas en un riguroso programa formativo suponen una mejora para el sistema sanitario, la profesión enfermera y la ciudadanía y la comunidad en la que vive. Aún existen muchos retos para que cada Consejería de Salud de cada comunidad haga posible que esta especialidad desarrolle todo su potencial de mejora de los cuidados

More the 30 years have passed since the first Royal Decree that regulated nursing specialties was published in 1987. It is the Royal Decree published in 2005 that really allowed the training development of part of the specialties recognized in it (family and community nursing, paediatric nursing, geriatric nursing and occupational health nursing) since the obstetric-gynaecological specialist nurses (midwives) and mental health specialists had long since already started their training processes through the resident internal nurse model, today, training in the specialty of family and community nursing has been implemented in all the autonomous communities, but has not had the same development in terms of the incorporation of specialists in the health institutions of the respective health services of the autonomous communities This circumstance is generating a great lack of motivation among community nurses, both specialists and those who hope to obtain the qualification through a specialty exam. Many of the objectives achieved to date have been made possible thanks to the work of the scientific societies of community nursing (Association of Community Nursing [AEC] and Federation of Associations of Community Nursing and Primary Care [FAECAP]), which have allowed progress to be made and the process not to be halted, although there are still many achievements to be made on which the aforementioned scientific societies continue to work. In a society in which nursing care is increasingly necessary and demanded, it must have greater consideration and position in health policies, since it has demonstrated its effectiveness and can be the model that allows the health system to be sustainable. Therefore, nurses who specialize in family and community nursing must cease constituting an opportunity and become a reality.

ABSTRACT Objective To explore the meaning of beliefs and practices related to family functioning and health care in a Venezuelan indigenous group. Materials and Methods A qualitative research study was conducted using a focused-ethnography (FE) method. The techniques implemented during data collection were focus group and observer-as-participant. Twenty-seven people distributed into four groups were included in the study. The qualitative content analysis method was selected for the interpretation stage. Results The most significant findings revealed that Jivi people's customs were mixed with modern practices. Cultural practices such as maternity and childbirth are processes in which the formal healthcare system has a secondary role. Moreover, participants showed flexibility in terms of responsibilities without association to sex. Conclusion Training of human resources with an intercultural approach is a measure that could strengthen the practices of ethnic groups in terms of healthcare.


RESUMEN Objetivo Explorar el significado de las creencias y prácticas relacionadas con la función familiar y el cuidado de salud en un grupo indígena venezolano. Métodos Se realizó una investigación cualitativa razonada con el método de Etnografía Enfocada (EE), las técnicas implementadas fue el grupo focal y la observación participante. Participaron N=27 personas distribuidas en cuatro grupos. El método seleccionado para la fase de interpretación fue el Análisis Cualitativo de Contenido. Resultados Los hallazgos mostraron que las creencias del pueblo Jivi se mezclaron con las prácticas modernas; las prácticas culturales como la maternidad y el parto se asumieron como un proceso en el cual el Sistema de Salud formal tiene una relación secundaria. Además, los participantes mostraron flexibilidad de responsabilidades sin referencia al género. Conclusión Una medida para fortalecer las prácticas de los grupos étnicos en términos de atención de salud es la capacitación de recursos humanos en el enfoque intercultural.

INTRODUCTION: Community nursing and midwifery is changing in response to a shift in care from hospital to home, brought about by increasing costs to care because of an aging population and increasing chronicity. Until now, community nursing positions and scope of practice has been dependent on service focus and location, which has led to the role being unclearly defined. Lack of appeal for a career in community practice and a looming workforce shortage necessitates a review into how community nursing and midwifery transition to practice is supported. METHODS: This review sought to identify, assess and summarize available evidence relating to transitioning into community nursing and midwifery practice as a speciality. A systematic review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses approach. A narrative synthesis was then undertaken on papers that examined community nursing and midwifery pathway perspectives which define, and enable or inhibit a contemporary pathway. Thematic analysis used a theoretical framework developed for early career and rapid transition to nursing specialty practice. RESULTS: There is a paucity of research that identifies community nursing and midwifery as a discreet scope of practice. Twelve papers were eventually included in the review. Verbatim findings were extracted from the papers and clustered into categories based on the chosen theoretical framework. Major themes were 'the self' (professional and personal); 'transition processes'; and, a 'sense of belonging'. Sub themes included narrative identifying inhibitors and enablers in each theme. DISCUSSION: No definition of community practice or pathway was identified in nursing, although midwifery was clearly defined. Community nursing practice was described as generalist in nature although specialist knowledge is required. Being part of the community in the professional sense and personal sense was considered important. The importance of transition was identified where pre-entry exposure to community practice was seen as important. Stages in transition to practice were recognised as pre-entry; incomer; insider; and, a sense of belonging. The process of transition should be planned and individualised acknowledging past experience whilst acknowledging the specialist nature of community-based practice.

Objetivo: Describir las prácticas de conocimiento y de enfermería en el contexto de la anticoncepción oral y construir de manera compartida, una tecnología educacional. Metodología: El estudio se realizó en 09 unidades de la Estrategia Salud de la Familia, que pertenece al municipio de Ananindeua, en la región metropolitana de Belém - Pará, con los participantes 21 enfermeras. La recolección de datos se llevó a cabo de agosto a noviembre 2015, a través de entrevistas individuales con semi-estructurada y análisis utilizó la técnica de análisis de contenido. Resultados: Se estableció contacto con las enfermeras que tienen un amplio conocimiento sobre la anticoncepción oral. Y sus prácticas, aunque relevante para lo que se recomienda para esta clientela, se ven obstaculizados por las condiciones de trabajo en sus unidades. Conclusión: Las enfermeras contribuyeron al contenido y la forma de la tecnología educativa, ya que consideraba pertinente para su uso como una herramienta importante para la educación sanitaria


Objetivo: Descrever conhecimentos e práticas de enfermeiros no contexto da contracepção oral e construir, de forma compartilhada, uma tecnologia educacional. Método: A pesquisa foi desenvolvida em nove Unidades da Estratégia Saúde da Família, pertencentes ao município de Ananindeua, localizado na Região Metropolitana de Belém/Pará, tendo como participantes 21 enfermeiros. A coleta de dados ocorreu no período de agosto a novembro de 2015, por meio de entrevista individual com roteiro semiestruturado, e, para análise, utilizou-se a técnica de análise de conteúdo. Resultados: Contatou-se que os enfermeiros têm amplo conhecimento com relação à contracepção oral. E suas práticas, embora pertinentes ao que está preconizado para essa clientela, encontram-se dificultadas pelas condições de trabalho em suas Unidades. Conclusão: Os enfermeiros contribuíram no conteúdo e forma da tecnologia educacional, pois consideraram pertinente a sua utilização como importante ferramenta para a educação em saúde


Objective: To describe nurses' knowledge and practices in the context of oral contraception and to construct, in a collaborative way, an educational technology. Methodology: The research was developed in 09 Units of the Family Health Strategy, belonging to the municipality of Ananindeua, located in the Metropolitan Region of Belém - Pará, with 21 nurses as participants. The data collection took place from August to November 2015 through individual interview with a semi-structured script and for the analysis the content analysis technique was used. Results: It was found that nurses have extensive knowledge regarding oral contraception. And its practices, although pertinent to what is recommended for this clientele, are hampered by the working conditions in its Units. Conclusion: Nurses contributed to the content and form of the educational technology, considering their relevance as an important tool for health education

BACKGROUND: Nurses exposed to community health nursing commonly encounter situations that can be morally distressing. However, most research on moral distress has focused on acute care settings and very little research has explored moral distress in a community health nursing setting especially among nursing students. AIM: To explore the moral distress experiences encountered by undergraduate baccalaureate nursing students in community health nursing. RESEARCH DESIGN: A descriptive qualitative design was employed to explore the community health nursing experiences of the nursing students that led them to have moral distress. PARTICIPANTS AND RESEARCH CONTEXT: The study included 14 senior nursing students who had their course in Community Health Nursing in their sophomore year and stayed in the partner communities in their junior year for 6 and 3 weeks during their senior year. ETHICAL CONSIDERATIONS: Institutional review board approval was sought prior to the conduct of the study. Self-determination was assured and anonymity and confidentiality were guaranteed to all participants. FINDINGS: Nursing students are vulnerable and likely to experience moral distress when faced with ethical dilemmas. They encounter numerous situations which make them question their own values and ideals and those of that around them. Findings of the study surfaced three central themes which included moral distress emanating from the unprofessional behavior of some healthcare workers, the resulting sense of powerlessness, and the differing values and mindsets of the people they serve in the community. CONCLUSION: This study provides educators a glimpse of the morally distressing situations that often occurs in the community setting. It suggests the importance of raising awareness and understanding of these situations to assist nursing students to prepare themselves to the "real world," where the ideals they have will be constantly challenged and tested.
